01 · Initial concern: a safety check appears to have been missed.
A manager receives a report suggesting the check did not happen at the expected time.
This example is fictional and is not based on an identifiable ORVIA client or confidential matter.

A manager receives a report suggesting the check did not happen at the expected time.

The timing is now disputed rather than simply absent.
It establishes when the entry was made, not necessarily when the physical check occurred.
The apparently simple account becomes less certain.
